29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical agents: a) New residential construction/Roof - On Or about February 10, 2025, the employer exposed employees to struck-by hazards while operating pneumatic nail guns, in that protective eyewear was not provided to employees operating these tools at 150 PSI while engaged in framing activities on a 10/12 pitched roof.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b): a) On the roof/New residential construction - On or about 02/10/2025, the employer exposed employees to fall hazards of 20-feet to the lower ground level, in that a complete fall protection system was not provided to employees engaged in roofing activities when lifelines were not connected to anchor points prior to the start of work.
