29 CFR  1926.102(a)(1):The employer  did not ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying objects while using a nail gun.  (a) On or about February 13, 2025, employees were using pneumatic nail guns to install framing lumber without wearing eye protection.

29 CFR  1926.501(b)(13):Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b), nor did the employer demonstrate that it is infeasible or creates a greater hazard to use these systems and develop and implement a fall protection plan which meets the requirements of paragraph (k) of section 1926.502:   (a) On or about February 13, 2025, employees doing roofing work of more than 6-feet from ground level without having fall protection.

29 CFR  1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet  above the upper landing surface to which the ladder was used to gain access:  (a) On or about February 13, 2025, employees performing roofing work and using an extension ladder that did not extend 3-feet above the landing.
