(a) In front of building 301 - On or about February 18, 2025, the employer exposed employees to a struck-by hazard, in that no hard hat was worn by an employee working on ground cleanup, under roofing activities above.

(a) On the roof of buildings 301 & 302 - On or about February 18, 2025, the employer exposed employees to a fall hazard, in that, the employer allowed the use of waist belts as part of a personal fall arrest system.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed.  (a) At the back of building 301 - On or about February 18, 2025, the employer exposed employees to a fall hazard, in that, the extension ladder used by employees for access to the roof was not extended at least three feet above the roof edge, with an eaves to ground fall distance of 19'.  Humberto Lopez Nava was previously cited for a repeat violation of this occupational safety and health standard or its equivalent standard 1926.1053(b)(1), which was contained in OSHA inspection number 1743624, citation number 2, item number 2 and was affirmed as a final order on June 17, 2024, with respect to a workplace located at 913 Highland Circle, Lot 58, Richmond Hill, GA 31324.  Humberto Lopez Nava was previously cited for a repeat violation of this occupational safety and health standard or its equivalent standard 1926.1053(b)(1), which was contained in OSHA inspection number 1728653, citation number 2, item number 2 and was affirmed as a final order on May 16, 2024, with respect to a workplace located at 102 Chillburn Court, Bloomingdale, GA 31302.  Humberto Lopez was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.1053(b)(1), which was contained in OSHA inspection number 1603707, citation number 1, item number 2 and was affirmed as a final order on October 14, 2022, with respect to a workplace located at 210 Crosswinds Drive, Rincon, GA 31326.
