29 CFR 1910.212(a)(3)(ii): The point of operation of machines whose operation exposes an employee to injury, shall be guarded. The guarding device shall be in conformity with any appropriate standards therefor, or, in the absence of applicable specific standards, shall be so designed and constructed as to prevent the operator from having any part of his body in the danger zone during the operating cycle. a. On or about February 12, 2025, at the front of the eccentric slotter machine, the guarding at the up and down was not adequate and did not prevent employees from reaching into the point of operation of the moving knife. b. On or about February 12, 2025, at the back of the eccentric slotter machine, the guarding at the up and down was not adequate and did not prevent employees from reaching into the point of operation of the moving knife.

29 CFR 1910.1030(f)(2)(i):Hepatitis B vaccination shall be made available after the employee has received the training required in paragraph (g)(2)(vii)(I) and within 10 working days of initial assignment to all employees who have occupational exposure unless the employee has previously received the complete hepatitis B vaccination series, antibody testing has revealed that the employee is immune, or the vaccine is contraindicated for medical reasons. On or about February 12, 2025, an employee cleaned blood from the eccentric slotter machine and was not offered the hepatitis B vaccination.
