Location: Job site;  On or about 02/20/2025, Brait Builders Corporation, as a controlling employer, did not ensure that frequent and regular inspections of the job site were performed by a competent person. As a result, subcontractor employees from N.B. Kenney Company, Inc. were exposed to hazards in the following instances:  Instance a: Slip/trip/fall hazards due to accumulation of snow and ice on the roof walking/working surfaces which was not effectively mitigated or eliminated prior to allowing work to commence. Instance b: Falls of 1 to 3-stories to a lower level where employees were not protected by the use of guardrail systems, safety net systems, or personal fall arrest systems from the roof where employees were working in slippery, snowy, and icy conditions. Instance c: HVAC curb openings being covered by Tyvek tarping which was not labeled "HOLE".
