29 CFR 1910.178(a)(5): The powered industrial truck was equipped with front-end attachments other than factory installed attachments, however the employer did not request that the truck be marked to identify the attachments and show the approximate weight of the truck and attachment combination at maximum elevation with load laterally centered: (a) On or about February 19, 2025, employees operated the Landoll Drexel forklift (Model SLT30/35 AC, Serial SLT30AC/2-2207A-13788) with a jib boom attached to the tines to move modules, which weigh approximately 500 to 800 pounds each, to and from the washer. The forklift was not appropriately marked to identify the new load data when the jib was utilized, thus exposing employees to struck-by and crushing hazards.

29 CFR 1910.178(l)(1)(i): The employer did not ensure that each powered industrial truck operator is competent to operate a powered industrial truck safely, as demonstrated by the successful completion of the training and evaluation specified in this paragraph (l): (a) On or about February 19, 2025, the employer exposed employees to crushing and struck-by hazards by authorizing employees who were not trained and certified to operate the Landoll Drexel (Model SLT30/35 AC, Serial SLT30AC/2-2207A-13788) forklift. This forklift is used to lift and move modules, which weigh approximately 500 to 800 pounds each, from the glass washers to the maintenance area.

29 CFR 1910.178(a)(4): Modifications or additions which affect capacity and safe operation of powered industrial truck were performed by the employer without the manufacturer's prior written approval: (a) On or about February 19, 2025, the employer exposed employees to crushing and struck-by hazards in that employees operated the Landoll Drexel forklift (Model SLT30/35 AC, Serial SLT30AC/2-2207A-13788) with a jib boom attached without obtaining the forklift manufacturer's written approval. This jib boom is attached to the tines of the forklift and is used to move modules, which weigh approximately 500 to 800 pounds each, to and from the washer. Prime Window Systems LLC was previously cited for a violation of this occupational safety and health standard , which was contained in OSHA inspection number 1582875, citation number 1, item number 3 , group letter a, and was affirmed as a final order on September 16, 2022, with respect to a workplace located at 12775 East 38th Avenue, Denver, CO 80239.
