29 CFR 1926.100(a):Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets: a) Restore Revamp LLC; on or about October 7, 2024, at the address of 2057 Napoleon Rd Bowling Green, OH. The employer did not ensure that employees were protected from head injury hazards by the use of protective hardhats while working on the ground as employees above actively installed roofing materials. Employees operating in this condition were exposed to struck by injuries from falling/flying objects such as but not limited to pneumatic nail guns and roofing materials.

On or about October 07, 2024, at the site location of Copper Beach apartment complex Bowling Green, OH: The employer did not ensure employees performing roofing work from an elevated surface were provided with an adequate form of fall protection. Employees performing working this condition were exposed to falls up to 23 ft. to the level below.
