29 CFR 1910.212(a)(3)(ii): Point(s) of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle(s): (a) Shop: On or before 12/27/2024 the employer did not ensure that the punch station on the Uni-Hydro 56 Ton Ironworker, model 56-14, serial number 3PE8596X was adequately guarded. The guard that was attached to the punch station did not prevent operators from placing their hands into the point of operation. This condition exposed employees to amputation hazards.
