29 CFR 1910.178(l)(1)(ii):The employer did not ensure that each operator had successfully completed the training required by paragraph (l), except as permitted by paragraph (l)(5), prior to permitting an employee to operate a powered industrial truck a) Ceva Logistics, US, Inc. located at 26230 Stokes Road, East Liberty, Ohio, 43319: On or about February 20, 2025, the employer did not ensure employees tasked with operating the forklifts were adequately trained in the operation of powered industrial vehicles. An evaluation of competency for employees operating forklift was not conducted and documented by their employer prior to operating the vehicles which exposes employees to struck by and crushing hazards.
