This condition exposed employees to caught-in and amputation hazards associated with the unexpected energizing, startup or release of stored energy.

29 CFR 1910.212(a)(3)(ii): Points of operation of machinery were not guarded or guards were not designed and constructed to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle(s): (a)Dakota Kitchen & Bath, Inc. at 4101 North Hainje Avenue, Sioux Falls, SD 57104: On or about and at times prior to February 20, 2025, the employer did not ensure the point of operation on the Jet Horizontal Metal Bandsaw located in the boxing area was effectively guarded to prevent employees from having any part of their body in the danger zone during specific machine operations. This hazard exposed employees to amputation hazards. (b)Dakota Kitchen & Bath, Inc. at 4101 North Hainje Avenue, Sioux Falls, SD 57104: On or about and at times prior to February 20, 2025, the employer did not ensure that the point of operation on the Lauderdale Hamilton Super Cut Upcut Saw located in the rough mill area was properly guarded to prevent employees from having any part of their body in the danger zone during specific machine operations. The dual hand controls provided by the manufacturer had been removed and replaced with a foot treadle. This hazard exposed employees to amputation hazards. An air hose used to clean the "Dovetailer" located in the "Drawers" department was measured at 100 p.s.i. This condition exposed employees to flying debris hazards as well as pulmonary embolisms. (b)Dakota Kitchen & Bath, Inc. at 4101 North Hainje Avenue, Sioux Falls, SD 57104: On or about and at times prior to February 20, 2025, the employer did not ensure that operating pressures for compressed air were less than 30 p.s.i, for cleaning purposes. An air hose used to clean the "Komo 1" CNC machine located in the "Parts" department was measured at 108 p.s.i. This condition exposed employees to flying debris hazards as well as pulmonary embolisms.
