29 CFR 1910.178(l)(4)(iii): An evaluation of each powered industrial truck operator's performance was not being conducted at least once every three years: a) On or about February 21, 2025, and at times prior, at 3512 6th Avenue SE, Decatur, AL 25603, Vision Wheel, Inc., did not ensure employees operating forklift (RAYMOND, Model # 425-C40TT with Serial # 425-14-43128) to move materials/products without having received refresher training at least once every three years. b) On or about February 21, 2025, and at times prior, at 3512 6th Avenue SE, Decatur, AL 25603, Vision Wheel, Inc., did not ensure employees operating forklift (HYSTER 50, Model # S50XM and Serial # D178V15063W) to move materials/products without having received refresher training at least once every three years. c) On or about February 21, 2025, and at times prior, at 3512 6th Avenue SE, Decatur, AL 25603, Vision Wheel, Inc., did not ensure employees operating forklift (RAYMOND, Model # 12-85-13 and Serial # 3214KB) to move materials/products without having received refresher training at least once every three years.
