29 CFR 1926.501(b)(13):Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b): (a) 340 Pleasantview Road, New Cumberland PA - On or about February 25, 2025, the employees who were working on a 4:12 pitch and 12:12 pitch 2nd story residential roof were not protected from falls of at least 10 feet to the lower level or 20 feet to grade. (b) 340 Pleasantview Road, New Cumberland PA - On or about February 25, 2025, the employees who were working on a 4:12 pitch 1st story residential roof were not protected from falls of at least 10 feet to grade. (c) 340 Pleasantview Road, New Cumberland PA - On or about February 25, 2025, the employees who were working on a 4:12 pitch 1 story detached garage were not protected from falls of at least 10 feet to the asphalt driveway. Abatement certification and documentation required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement. Abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic or video evidence of abatement or other written records.

29 CFR 1926.503(a)(1):The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards: (a) 340 Pleasantview Road, New Cumberland PA - On or about February 25, 2025, the employer did not ensure that employees potentially exposed to falls were trained to recognize the hazards of falling. Abatement certification required within 10 days after abatement date.
