29 CFR 1910.141(a)(3)(i): The employer did not ensure that all places of employment were kept clean to the extent that the nature of the work allows: 462 Main Street, Agawam, MA: On February 25, 2025, and at times prior, employees were exposed to the hazards of mold growth along the ceiling tiles throughout the hallways of the Memory Care unit and along the hallways to the staff break room.
