Location: 85 Holbrook Avenue, Wellfleet, MA On or about February 25, 2025, employees were exposed to struck by hazards while using a pneumatic nail gun where eye protection was not utilized.

Location: 85 Holbrook Avenue, Wellfleet, MA On or about February 25, 2025, employees were exposed to fall hazards while accessing the scaffold where adequate access was not utilized
