On or about January 13, 2025, employees entered bin number 2 to clean out the wheat from the bin.   2. On February 24, 2025, an employee was operating the sweep auger on bin number 4 to clean out the corn. The employee then entered the bin.

On or about January 13, 2025, employees entered bin number 2 without the sweep auger being adequately disconnected, locked out and tagged, blocked-off, or otherwise prevented from operating by other equally effective means or methods to clean out the wheat from the bin.  2. On February 24, 2025, an employee was operating the sweep auger on bin number 4 to move and/or clean out the corn. The employee entered the bin without the sweep auger being disconnected, locked out and tagged, blocked-off, or otherwise prevented from operating by other equally effective means or methods.
