On or about February 25, 2025, employees engaged in rendering activities in the Rendering Department Truck Loadout Section of Tyson Fresh Meats, Inc. located at 1131 Dakota Avenue, Dakota City, Nebraska were exposed to trip and fall and caught-in hazards in that the employer did not ensure the grated walking working surface near and/or around the sump pit pump start/stop button was continuous and free from gapping.  Specifically, there was an opening between the two grated pieces below the start/stop button measuring approximately 21 inches by 8 inches (length by width).
