Vides Builders, Inc. and Ozark Organics LP employees were wearing N95 respirators when cleaning and working around grain dust without being given appendix D training.

Prior to employees entering, the employer did not disconnect, lock out and tag out, block-off, or otherwise prevent the sweep auger from operating by other equally effective means or methods. 1. On or about January 13, 2025, employees entered bin number 2 without the sweep auger being adequately disconnected, locked out and tagged, blocked-off, or otherwise prevented from operating by other equally effective means or methods to clean out the wheat from the bin. 2. On February 24, 2025, an employee was operating the sweep auger on bin number 4 to move and/or clean out the corn. The employee entered the bin without the sweep auger being disconnected, locked out and tagged, blocked-off, or otherwise prevented from operating by other equally effective means or methods.
