29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: a) Warehouse/Production floor - On or about 2/25/2025, and at times prior, the employer exposed employees to amputation, chemical and electrical shock hazards in that, the employer failed to ensure the hazardous energy control lockout/tagout procedures were developed, documented, and implemented for the employees during servicing and maintenance of equipment, such as but not limited to dryer, regenerative thermal oxidizer and shredder machines.

29 CFR 1910.1200(h)(3)(iii): Employee training did not include the measures employees can take to protect themselves from chemical hazards, including specific procedures the employer had implemented to protect employees from exposure to hazardous chemicals, such as appropriate work practices, emergency procedures and personal protective equipment to be used: a) Throughout the warehouse - On or about 02/09/2025, and at times prior, the employer exposed employees to chemical hazards in that, the employer failed to ensure that employees training included specific procedures to protect employees from exposure to hazardous chemicals, while employees were working with Rechargeable Lithium ion battery that can produce hazardous chemicals such as, but not limited to carbon monoxide, hydrogen fluoride (HF) gas and hydrogen gas.
