29 CFR 1910.147(d): The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6): (a) Big Panning Room: On or about February 26, 2025, employees performing sanitation work were exposed to machine hazards associated with the unexpected startup of moving parts and in-going nip points during the removal of mechanical power-transmission apparatus guard panels, of the #2 Panning machine, #4 Panning machine, #5 Panning machine and #6 Panning machine, located in the Big Panning room. The employer failed to implement energy control application steps as LOTO devices were not applied to the electrical disconnect [per the 1910.147(d)(4) requirements]. As a result, the remaining applicable energy control elements, involving dissipation of residual energy [(d)(5)(i)], and verification of isolation [(d)(6)], were not implemented to protect employees from machine servicing hazards. No abatement certification or documentation is required for this item.
