29 CFR  1926.21(b)(2): The employer did not instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his/her environment to control or eliminate any hazards or other exposure to illness or injury:   a.  On or about February 25, 2025, at the above addressed jobsite, employees were not trained in the recognition and avoidance of unsafe conditions applicable to the work environment and how to eliminate their exposure to jobsite hazards.

29 CFR  1926.454(a): The employer did not have each employee who performed work while on a scaffold trained by a person qualified in the subject matter to recognize the hazards associated with the type of scaffold being used and to understand the procedures to control or minimize those hazards.  a.  On or about February 25, 2025, at the above addressed jobsite, the employer did not train each employee performing work from a JLG 800AJ Articulating Boom Lift on the hazards associated with using and operating an aerial lift, nor did the employer train employees on the nature of any electrical hazards in the work area and correct procedures for dealing with electrical hazards.

29 CFR  1926.416(a)(1): The employer permitted employees to work in close proximity to energized, unguarded, and ungrounded electric power circuits that the employee could contact in the course of their work:  a.  On or about February 25, 2025, at the above addressed jobsite, employees installing exterior metal framing from a JLG 800 AJ Articulating Boom Lift were required to work within close proximity to energized power lines, thereby exposing the employees to electrical hazards.

29 CFR  1926.416(a)(3): The employer began work without determining if employees could contact any part of an energized circuit and did not post warning signs and advise employees of the location of the circuit, hazards, and protective measures they need to take.  a.  On or about February 25, 2025, at the above addressed jobsite, the employer did not ensure that the performance of the work would not bring the employees and/or machine into physical or electrical contact with any part of the energized overhead power lines, nor did the employer post warning signs and advise employees of the hazards and protective measures to be taken, thereby exposing the employees to electrical hazards.
