29 CFR 1926.417(b): Equipment or circuits that were de-energized were not rendered inoperative: (a) Mayo Clinic Hospital, 2nd floor of Wish Art Building, environmental services/janitorial closet (room no. 2416J); On or about February 12, 2025, and dates prior, circuit breaker numbers 1, 2, 5, 15, 17, and/or 19 located in electrical panel 2XC2-A were not rendered inoperative and/or did not have energy isolation (lockout) devices attached to them to ensure the breakers could not be turned on (reset) while electrical employees installed additional 120 VAC receptacles in each of the 12 rooms being renovated in the Medical Ready Discharge Area.
