29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury:  a) New residential construction located at The Enclave at Cooper Creek Subdivision, 2527 Swan Lake Dr., Grayson, GA 30017, Front Roof: On or about 02/25/2025, the employer exposed two employee to struck-by hazard while operating pneumatic nail guns, in that protective eyewear was not provided to employees engaged in framing activities operating these tools at 120 PSI.

29 CFR  1926.501(b)(13):Each employee engaged in residential construction activities 6 feet (1.8 m)  or more above lower levels was not protected by guardrail systems, safety net systems, personal fall arrest systems, or an alternative fall protection measure under another provision of paragraph 1926.501(b), nor did the employer demonstrate that it is infeasible or creates a greater hazard to use these systems and develop and implement a fall protection plan which meets the requirements of paragraph (k) of section 1926.502:   a) New Construction at The Enclave at Cooper Creek Subdivision, 2527 Swan Lake Dr., Grayson, GA 30017, Front Roof: On or about 02/25/2025, the employer exposed one employee to fall hazards while working on the top plate of a 8/12 pitched roof with an eave height of approximately 19-feet, 5-inches to the lower ground level, in that a fall protection system was not provided while the employee was engaged in installing trusses.  b) New Construction at The Enclave at Cooper Creek Subdivision, 2527 Swan Lake Dr., Grayson, GA 30017, Second level window rough opening: On or about 02/25/2025, the employer exposed one employee to fall hazards while using a window sill as a walking/working surface at heights of approximately 14-feet, 11-inches to the lower ground level, in that a fall protection system was not provided while the employee was nailing off plywood sheathing.

29 CFR 1926.1053(b)(1): Where (a) portable ladder(s) was/were used for access to an upper level landing surface and the ladder's length did not allow the ladder side rails to extend at least 3 feet (0.9 m) above the upper landing surface being accessed, the ladder was not secured at its top to a rigid support that will not deflect, and/or a grasping device was not provided to assist employees in mounting and dismounting the ladder.    a) New residential construction located at The Enclave at Cooper Creek Subdivision, 2527 Swan Lake Dr., Grayson, GA 30017, Front roof: On or about 02/25/2025, and at times prior, the employer exposed five employees to fall hazards from an eave height of approximately 19 feet, 5 inches to the lower level, in that the ladder side rails of the extension ladder did not extend at least 3-feet past the roof landing to allow for safe access on and off the roof.
