29 CFR 1926.502(d)(15)(ii): Anchorages used for attachment of personal fall arrest systems were not designed, installed and used under the supervision of a qualified person: a) Jobsite: 933 N Penn St, Philadelphia - On or about February 25th, 2025, the employer did not ensure the anchor used by an employee as part of his personal fall arrest system while installing an electrical box on the ceiling over an open and unprotected 5th floor apartment building balcony, approximately 45 feet above the ground level, was installed under the supervision of a qualified person.
