29 CFR 1910.147(d): The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6): a.) Warehouse - On or about February 14, 2025, and days prior to, employees were exposed to machine hazards associated with moving parts when changing blades on the Marvel Series PA22MPC Horizontal Metal Cutting Bandsaw. The employer failed to implement energy control application steps as the machine was not shut down or turned off to perform the servicing work [per the 1910.147(d)(2) requirements]. As a result, the remaining applicable energy control elements, involving machine isolation [(d)(3)], LOTO device application [(d)(4)], dissipation of residual energy [(d)(5)((i)], and verification of isolation [(d)(6)], were not implemented to protect employees from machine servicing hazards. b.) Warehouse - On or about February 22, 2025, and days prior to, employees were exposed to machine hazards associated with moving parts when changing blades on the Marvel Series PA22MPC Horizontal Metal Cutting Bandsaw. The employer failed to implement energy control application steps as the machine was not shut down or turned off to perform the servicing work [per the 1910.147(d)(2) requirements]. 29 CFR 1910.176(b): Material stored in tiers was not stacked, blocked, interlocked or limited in height so that it was stable and secure against sliding and collapse: (a) Warehouse, North End - On or about February 20, 2025, metal pipes were not stacked and blocked in a stable and secure manner, thereby exposing employees to struck-by hazards. While using an overhead crane (#6) to pull a 16" double random length (DRL) carbon steel pipe from the roller table, an employee sustained three (3) broken toes and one (1) dislocated toe when the lifted pipe struck a 42' X 12.750"X.188 pipe, causing the pipe to roll from the table and strike the employee on the left foot and ankle.
