On or about February 27, 2025, at the above addressed jobsite an employee performing commercial masonry  operations, from a third floor, was not protected from falls, thereby exposing the employee to fall hazards.  b.  On or about February 27, 2025, at the above addressed jobsite an employee performing commercial masonry  operations, from a second floor window opening, was not protected from falls, thereby exposing the employee to fall hazards.  c.  On or about February 27, 2025, at the above addressed jobsite  employees performing commercial masonry  operations, were exposed to falls hazards, due to a south elevator shaft that did not have guardrails.  d.  On or about February 27, 2025, at the above addressed jobsite  employees performing commercial masonry  operations, were exposed to falls hazards, due to a north elevator shaft that did not have guardrails.

On or about February 27, 2025, at the above addressed jobsite, an employee conducting masonry operations to include but not limited to; the removal/cutting up of masonry brick that contained Crystalline Quartz Silica, by use of a of a Stihl Partner Saw.  The employer did not implement engineering controls, thereby exposing employee to the hazards associated with silica.
