29 CFR 1926.651(k)(1): Daily inspections of excavations, the adjacent areas, and protective systems were not made by a competent person for evidence of a situation that could have resulted in possible cave-ins, indications of failure of protective systems, hazardous atmospheres, or other hazardous conditions: a) On or about February 27, 2025, an employee was exposed to cave-in hazards while working in an unprotected trench with vertical walls of soil class type "C" that was measured to be 7 feet 10 inches in depth, 25 feet long, and 11 feet wide, and without a stairway, ladder, ramp or other safe means of egress within 25 feet. The competent person onsite did not perform an inspection of the excavation.
