29 CFR  1910.37(b)(6): Each exit sign was not illuminated to a surface value of at least five foot-candles (54 lux) by a reliable light source and be distinctive in color:  (a)Hills Product Group, Inc., at 1421 Laurel Street, Whitewood, SD 57793: On or about and at times prior to February 25, 2025, the employer did not ensure that an emergency exit door was illuminated to a surface value of at least five foot-candles (54 lux) be a reliable light source and be distinctive in color for an exit door located in the "Receiving Area" building.  This condition exposed employees to egress hazards in the event of an emergency.

29 CFR  1910.147(c)(4)(ii): Procedures did not clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, and the means to enforce compliance including, but not limited to, 29 CFR 1910.147(c)(4)(ii)(A), (c)(4)(ii)(B), (c)(4)(ii)(C) and (c)(4)(ii)(D):  (a)Hills Product Group, Inc., at 1421 Laurel Street, Whitewood, SD 57793: On or about and at times prior to February 25, 2025, the employer did not develop machine specific procedural steps for shutting down, isolating, placement, removal, and transfer of lockout devices and specific requirements for testing machines or equipment to determine and verify the effectiveness of lockout devices and other energy control measures to prevent employee injury during maintenance or servicing of multi-energy source powered equipment to include, but not limited to, the "Guardrail Machine" located in the "Receiving Area"  This condition exposed employees to caught-in and amputation hazards associated with the unexpected energizing, startup or release of stored energy.

29 CFR  1910.178(a)(6):The employer did not ensure that all nameplates and markings were in place:  (a)Hills Product Group, Inc. at 1421 Laurel Street, Whitewood, SD 57793: On or about and at times prior to February 25, 2025, the employer did not ensure that a CAT Diesel forklift located in the "Treatment Plant" was marked to identify load and lifting capabilities.  This condition exposed employees to crush-by hazards associated with unexpected forklift performance.  (b)Hills Product Group, Inc. at 1421 Laurel Street, Whitewood, SD 57793: On or about and at times prior to February 25, 2025, the employer did not ensure that a CAT Diesel forklift located in the "Garage Area" was marked to identify load and lifting capabilities.  This condition exposed employees to crush-by hazards associated with unexpected forklift performance.
