29 CFR 1910.147(c)(4)(i): Procedures shall be developed, documented and utilized for the control of potentially hazardous energy when employees are engaged in the activities covered by this section. a. On or about February 28, 2025, the employer failed to develop, document and utilize equipment specific procedures for authorized employees who perform maintenance activities and utilize lockout/tagout on equipment not limited to Greenlease line machine numbers 483, 555, and 575. b. On or about February 28, 2025, the employer failed to develop, document and utilize equipment specific procedures for authorized employees who perform maintenance activities and utilize lockout/tagout on equipment not limited to Greenlease department/area spot welders. c. On or about February 28, 2025, the employer failed to develop, document and utilize equipment specific procedures for authorized employees who perform maintenance activities and utilize lockout/tagout on equipment not limited to Turbo department/area spot welders.

29 CFR 1910.255(b)(4):All press welding machine operations, where there is a possibility of the operator's fingers being under the point of operation, were not effectively guarded by the use of a device such as an electronic eye safety circuit, two hand controls or protection similar to that prescribed for punch press operation. (a) On or about February 28, 2025, the employer did not ensure that the spot-welders located in the Turbo department/area were effectively guarded at the point of operation to prevent employees from reaching into the hazardous area. Employee(s) hand feed material directly into the point of operation during production. (b) On or about February 28, 2023, the employer did not ensure that the spot-welder located in the Greenlease department/area were effectively guarded at the point of operation to prevent employees from reaching into the hazardous area. Employee(s) hand feed material directly into the point of operation during production.
