29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access: a) On or about February 27, 2025, at 8140 South Tropical Trail, Merritt Island FL: the employer allowed employees to use extension ladders to gain access to the roof and the rails of the ladders did not extend at least three feet above the roof landing to provide safe access exposing employees to a fall hazard of up to 15 feet to the ground below.
