Location: Job site; On or about 02/28/2025, employees were permitted to use rope lifelines while working on a residential roof 11 feet above a lower level, despite the lifelines showing signs of wear and deterioration. These lifelines were not removed from service prior to the start of work.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b). Location: Job site; On or about 02/28/2025, employees were allowed to engage in roofing activities without the use of fall protection on a residential roof that was 11-feet (rear of residence) above the ground below. Empire Monsters, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard (29 CFR 1926.501(b)(13)), which was contained in OSHA inspection number 1615161, citation number 2, item number 1 and was affirmed as a final order on 11/03/2022, with respect to a workplace located at 31 Church Street, Belmont, NH 03220.
