29 CFR 1926.20(b)(2): A program that provided for frequent and regular inspections of the job sites, materials, and equipment to be made by competent persons was not effectively implemented: a) Job site, 1200 Chess Street, Monongahela, PA: On or about February 28, 2025 during construction activities, the employer did not ensure an appropriate system was in placed to identify and correct safety and health hazards. Deficiencies noted during the inspection included, but were not limited, not ensuring proper fall protection was in use at all times on the roof, failing to ensure that electrical cords were not exposed to damage and floor holes were not provided with covers.
