29 CFR 1910.178(a)(4): Modifications and additions which affect capacity and safe operation of powered industrial truck were performed by the employer without the manufacturer's prior written approval: a) Lyons & Sons Inc. - Warehouse Area: Employees were operating the Nissan Propane Forklift Model # MCPL 02A25LV, S/N CPL-02-9P70V4, with the Cascade Sheet-Sav Push/Pull lift truck fork attachments, S/N 35F-PB-195-T-401, to slide and lift materials without obtaining the forklift manufacturer's written approval for the use of the attachments, occurred on or about 03/03/2025.
