29 CFR  1926.20(b)(2):The employer did not initiate and maintain programs which provided for frequent and regular inspections of the job site, materials and equipment to be made by a competent person(s).  (a) On or about March 4, 2025, the employer had not ensured that frequent jobsite inspections were conducted by a competent person(s) to identify and correct common safety hazards that employees were exposed to. Hazards identified during the inspection included but were not limited to, fall protection not being utilized while working at heights, installation of ladder used to access and egress the residential roof, and safety glasses not worn while using pneumatic nail guns.

29 CFR  1926.501(b)(13):Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b).  (a) On or about March 4, 2025, employees conducting roofing work on an existing residential roof with a 6:12 pitch, were not protected from falling to the lower level. Exposing employees performing roofing work to a fall hazard of approximately 18.5 feet to the ground below.  (b) On or about March 4, 2025, employees conducting roofing work on an existing residential garage, were not protected from falling to the lower level. Exposing employees performing roofing work to a fall hazard of approximately 9.5 feet to the concrete below.
