Boise Cascade Building Materials Distribution, LLC dba Boise Cascade Company - on or about 3/4/25, and at times prior thereto, overhead guard covers on forklifts including, but not limited to, Toyota forklift (serial #10292, Model #BFG5OU) were not included in the pre-shift examination inspection. ABATEMENT CERTIFICATION REQUIRED: Abatement certification is required within 10 days after the abatement date noted on the citation (see below). This certification shall include a statement that abatement is complete, the date abatement was completed, method of abatement, and state that employees and their representatives were informed of this abatement.
