29 CFR  1926.501(b)(13):Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):  a. On or about March 4, 2025, at Lot 285: employees were exposed to 11 foot 6 inch fall hazards while installing concrete blocks,  in that,  fall protection was not used.   LVQ Solutions Group LLC, was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(13), which was contained in OSHA inspection number 1751412, citation number 1, item number 1 and was affirmed as a final order on 09-19-2024, with respect to a workplace located at 4156 Jesperson st, Lot 95, Apopka, FL 32703.

On or about March 4, 2025, at Lot 285: the employer did not prepare and maintain a written forklift training program certification for employees exposed to struck-by hazards from a Sky Trak 6042 class 7 power industrial truck.
