(a) Ammonia refrigeration system, machine room: On or about 3/4/25, the employer did not document that the machine room ventilation complied with the requirements of the chosen RAGAGEP, IIAR-2 2021 6.14.3.4, in that continuous and emergency ventilation did not discharge at the required velocity and/or design and rate of ventilation was not documented. (b) Ammonia refrigeration system, engine room: On or about 3/4/25, the employer did not document that the Engine Room complied with the requirements of the chosen RAGAGEP, IIAR-2 2021 6.7.2, in that an emergency eyewash and shower unit was not installed in a nearby location outside the machine room. (c) Ammonia refrigeration system, dock area: On or about 3/4/25, the employer did not document that ammonia refrigeration pipes and equipment complied with the chosen RAGAGEP, IIAR-2 2021 7.2.4, in that overhead evaporator units, including but not limited to AHU 25, were not protected from physical damage by forklifts operating nearby.
