29 CFR 1910.134(k)(6):The employer did not provide the basic advisory information on respirators, as presented in Appendix D of 29 CFR 1910.134, in written or oral format to employees who wear respirators when such use was not required by the employer: (a) Icon Equipment Distributors, Inc, 300 Ryders Ln East Brunswick, NJ 08816 On or about March 5, 2025, and thereafter, employee(s) who voluntarily wear N-95 respirators were not provided the basic advisory information including care, cleaning and limitations of use of respirators, as presented in Appendix D of 29 CFR 1910.134.
