29 CFR  1910.67(c)(2)(v): A body belt was not worn and lanyard was not attached to the boom or basket when working from an aerial lift:  On or about March 5, 2025, at 1695 NW 9th Avenue, Miami, Florida, an employee was exposed to ejection and fall hazards of approximately 28 feet in height while pressure wash cleaning windows from an articulating aerial bucket without wearing and attaching a personal fall arrest or restraint system to the bucket's anchorage point.
