29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative: a) On or about 03/05/2025 throughout the establishment; where employees perform servicing and/or maintenance activities such as, but not limited to: changing saw blades and grinding wheels. The employer has not developed machine-specific energy control procedures, nor has the employer trained or periodically reviewed employees performing covered operations where the employees were exposed to potentially hazardous energy sources and amputation hazards. ABATEMENT DOCUMENTATION REQUIRED

29 CFR 1910.147(c)(4)(i): Procedures were not utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: a) On or about 03/05/2025 in the Tool Room; employees perform servicing and/or maintenance activities such as, but not limited to: changing grinding wheels on the surface grinder 612 and 618 Micromaster without utilizing Lockout /Tagout (LOTO) procedures. Employees were exposed to amputation hazards. b) On or about 03/05/2025 in the Cleaning & Saw Area; employees perform servicing and/or maintenance activities such as, but not limited to: changing saw blades on the COSEN Saw AH-250H without utilizing Lockout /Tagout (LOTO) procedures. Employees were exposed to amputation hazards. ABATEMENT CERTIFICATION REQUIRED
