29 CFR  1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  (a)  On the roof of a residence located at 3715 14th St W Lot 177, Bradenton, FL 34205: On or about March 5, 2025; the employer exposed employees to an approximate 7-ft. fall hazard, in that the extension ladder did not extend 3-ft. above the landing surface.

29 CFR  1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   (a)  On the roof of a residence located at 3715 14th St W Lot 177, Bradenton, FL 34205: On or about March 5, 2025; employees were exposed to a 17- foot fall hazard while performing roofing activities on a 4 in 12 pitched (vertical to horizontal) roof, in that, fall protection was not used.  The W&Y Construction Services LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR  1926.501(b)(13), which was contained in OSHA inspection number 1701006, citation number 1, item number 1 and was affirmed as a final order on 11/17/2023, with respect to a workplace located at 27840 Whales St, Wesley Chapel, FL, 33544.

29 CFR  1926.503(b)(1):The employer did not verify compliance with paragraph (a) of this section by preparing a written (training) certification record including the name or other identity of the employee trained, the date(s) of the training, and the signature of the person who conducted the training or the signature of the employer.  (a)  On the roof of a residence located at 3715 14th St W Lot 177, Bradenton, FL 34205: On or about March 5, 2025; the employer exposed employees to fall hazards, in that the completion of fall protection training for the employees was not certified by the employer.  The W&Y Construction Services LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR  1926.503(b)(1), which was contained in OSHA inspection number 1701006, citation number 2, item number 1 and was affirmed as a final order on 11/17/2023, with respect to a workplace located at 27840 Whales St, Wesley Chapel, FL, 33544.
