29 CFR 1910.305(g)(2)(iii): Flexible cords and cables were not connected to devices and fittings so that strain relief was provided that would prevent pull from being directly transmitted to joints or terminal screws: 450 Shed: On or about March 5, 2025, and at times prior thereto, employees utilizing the plastic wrap machine, Phoenix Model # PHP 2300, Serial #17064244, were exposed to electrical shock in that the machine was not connected so that strain relief was provided. The electrical connection under the electrical box was pulled away, exposing the inner wire, and held together with zip ties.

29 CFR 1910.101(b):The in-plant handling, storage, and utilization of all compressed gases in cylinders, portable tanks, rail tankcars, or motor vehicle cargo tanks were not in accordance with Compressed Gas Association Pamphlet P-1-1965, which is incorporated by reference as specified in CFR 1910.6: 450 Shed, East end of building, next to yellow cage: On or about March 5, 2025, and at times prior thereto, employees working in the warehouse were exposed to struck-by hazards in that one forklift propane tank was upright and unsecured from displacement.
