29 CFR 1910.178(m)(7): Brakes shall be set, and wheel blocks shall be in place to prevent movement of trucks, trailers, or railroad cars while loading or unloading. Fixed jacks may be necessary to support a semitrailer during loading or unloading when the trailer is not coupled to a tractor. The flooring of trucks, trailers, and railroad cars shall be checked for breaks and weakness before they are driven onto.  On or about February 28, 2025, the employer did not ensure that wheel blocks (chocks) were placed to prevent the movement of trucks while loading or unloading, nor were alternative methods of preventing truck movement used. Forklift Operators entering and/or exiting the truck were exposed to the unexpected movement of trucks being loaded and/or unloaded which could in serious injury.  Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).

29 CFR 1910.305(b)(1)(i): Conductors entering cutout boxes, cabinets, or fittings shall be protected from abrasion, and openings through which conductors enter shall be effectively closed.  On or about March 5, 2025, the employer did not ensure that conductors entering the junction/pull box leading to the dock lock outside for Dock 26 were provided with a fitting to protect from abrasions. The conduit was not secured to the junction/pull box, and the edges of the conduit were sharp and rusted. The individual conductors ran through the conduit are not protected from abrasion and are also exposed to outside elements (rain, snow, etc.), exposing employees to electrical shock hazards.  Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).

29 CFR 1910.305(b)(2)(i): All pull boxes, junction boxes, and fittings shall be provided with covers identified for the purpose. If metal covers are used, they shall be grounded. In completed installations, each outlet box shall have a cover, faceplate, or fixture canopy. Covers of outlet boxes having holes through which flexible cord pendants pass shall be provided with bushings designed for the purpose or shall have smooth, well-rounded surfaces on which the cords may bear.  On or about March 5, 2025, the employer did not ensure that the pull box/junction box leading to the dock locks outside for Dock 25 and Dock 26 was provided with covers. The individual conductors ran through the conduit are exposed to outside elements (rain, snow, etc.), exposing employees to electrical shock hazards.  Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).

29 CFR 1910.178(a)(6): The user shall see that all nameplates and markings are in place and are maintained in a legible condition.  On or about March 5, 2025, the employer did not ensure markings, on the Toyota 8FBE15U Electric Forklift s/n: 10512 (Forklift #16-4), were legible. The caution, warning, and inspection/maintenance checklist labels were illegible and/or peeling off.  Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).
