29 CFR  1910.147(a)(2)(iii)(A):Work on cord and plug connected electric equipment for which exposure to the hazards of unexpected energization or start up of the equipment was not controlled by the unplugging of the equipment from the energy source and by the plug being under the exclusive control of the employee performing the servicing or maintenance.  Plant: Not all equipment was unplugged during cleaning and sanitation operations, including but not limited,  to Marvel portion machines, Unifood Techni Skinners, Curio C-20111 De Boner, and Scalers machines.  Machine specific lockout-tagout procedures were not developed for machines that employer reported were not feasible to unplug.

29 CFR  1910.212(a)(3)(ii):Point of operation guards were not designed and constructed as to prevent the operator from having any part of their body in the danger zone during the operating cycle:    Dock Area: The cutting blade on the RUNI Screw Compactor was not guarded, exposing employees to laceration hazards.

29 CFR  1910.95(d)(1):When information indicated that any employee's exposure equaled or exceed the 8-hour time-weighted average of 85 decibels, the employer did not develop and implement a monitoring program:   Plant: Full shift noise monitoring had not been conducted to determine if levels exceeded the 8-hour time weighted action level of 85 dBA, where noise levels ranged from 95 to 103 decibels at the RUNI Screw Compactor and noise levels ranged from 86 to 89 dBA at the at the Curio C-2011 fish machine.
