29 CFR 1910.1025(h)(1):All surfaces were not maintained as free as practicable of accumulations of lead: On or about March 6, 2025, employees utilizing the change room were exposed to health hazards due to surfaces not being maintained as free as practicable of accumulations of lead: a. Wipe sample taken on bench in clean side of change room showed lead exposure of 6.6707 micrograms per 100cm2. b. Wipe sample taken on employee locker in clean side of change room showed lead exposure of 2.7986 micrograms per 100cm2.

29 CFR 1910.1053(i)(1)(i):The employer did not make medical surveillance available at no cost to each employee, and at a reasonable time and place, for each employee who has been occupationally exposed to respirable crystalline silica at or above the action level for 30 or more days per year: On or about March 6, 2025, an employee conducting melt, pour, and casting activities was exposed to an 8-hour time weighed average (TWA8) of 29.8 micrograms/cubic meter (ug/m3) of respirable crystalline silica (RCS), approximately 1.2 times the Action Level of 25 ug/m3. The exposure level was derived from samples collected over a 300-minute sampling period, with zero exposure assume for the period of 180 minutes and medical surveillance was not made available at no cost to each employee, and at a reasonable time and place, for each employee who has been occupationally exposed to respirable crystalline silica at or above the action level for 30 or more days per year.
