29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: 500 Fortune Blvd, Milford, MA: On and before March 6, 2025, the employer did not require employees to lock out or unplug the electrical mixer when changing the filter bag on tank 6A. On February 26, 2025, an employee received a fingertip amputation injury when the tank 6A filter being installed became wrapped around the shaft of the adjacent operating mixer.
