29 CFR  1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section:  a) On or about 03/06/2025, throughout the establishment;  employees were exposed to lacerations when Lockout/Tagout procedures were not utilized while employees changed blades on band saws by only turning off the electrical source and did not apply a lock.  b) On or about 03/06/2025, throughout the establishment; employees were exposed to lacerations when Lockout/Tagout procedures were not utilized while employees changed grinding wheels throughout the shop and did not use locks.   c) On or about 03/06/2025, throughout the establishment; employees were exposed to electric shocks when Lockout/Tagout procedures were not utilized while employees doing electrical repairs/service on a transformer did not utilized lockout/tagout procedures and only  shut off power at the bus box.  NO ABATEMENT DOCUMENTATION REQUIRED.

29 CFR 1910.147(c)(7)(i): The employer did not provide training to ensure that the purpose and function of the energy control program are understood by employees and that the knowledge and skills required for the safe application, usage, and removal of the energy controls are acquired by employees:  a) On or about 03/06/2025, throughout the establishment; where employees perform servicing and/or maintenance activities such as, but not limited to: changing blades on bandsaws, replacing wheels on grinding machines, and servicing transformers. Employees were not trained to recognize hazardous energy sources in the workplace and the means and methods (procedures) used to control those hazardous energy sources during servicing and/or maintenance of the machines and equipment in the workplace. 29 CFR  1910.305(g)(1)(i):Flexible cords and cables were not approved for conditions of use and location:  a) On or about 03/06/2025, at the Mazak Nexus 350-II machine; Employees were exposed to electrical hazards when an extension cords was used to power the oil skimmer and a hand tool battery charger.   b) On or about 03/06/2025, in the GRT-New part of shop area; Employees were exposed to electrical hazards when an extension cord was dropped from the ceiling at the Kellenburg machine and used to run a coolant pump.   ABATEMENT CERTIFICATION REQUIRED.

29 CFR  1910.1200(f)(6)(ii): Except as provided in 29 CFR 1910.1200(f)(7) and 29 CFR 1910.1200(f)(8), the employer did not ensure that each container of hazardous chemicals in the workplace was labeled, tagged or marked with the product identifier and words, pictures, symbols, or combination thereof, which provide at least general information regarding the hazards of the chemicals and which, in conjunction with the other information immediately available to employees under the hazard communication program, would provide employees with the specific information regarding the physical and health hazards of the hazardous chemical  a) On or about 03/06/2025, throughout the establishment; employees were exposed to eye and skin irritation, when 5 gallon buckets, and 16 ounce bottles that contained chemicals such as but not limited to; NuCool 3311 a metal working coolant were not marked with an identity and an appropriate hazard warning label.
