29 CFR 1904.4(a): The employer did not record each work-related fatality, injury or illness case that resulted in the general recording criteria on the OSHA Form 300 or equivalent. a) Jobsite: On or about November 5, 2024, the employer did not record a work-related injury, where an employee slipped on ice and twisted his ankle, resulting in days away from work. b) Jobsite: On or about December 5, 2024, the employer did not record a work-related injury, where an employee lacerated his thumb with a razor knife and received stitches, resulting in approximately three weeks out of work. c) Jobsite: On or about November 2024, the employer did not record a work-related injury, when an employee lacerated their thumb on a piece of steel, resulting in stitches.

29 CFR 1910.334(a)(2)(ii): There was a defect or evidence of damage that could have exposed an employee to injury and the defective or damaged item was not removed from service. a) Garage: On or about March 7, 2025, the employer did not ensure extension cords were visually inspected prior to each shift for defects, to include the cord that was plugged into a power source outside the maintenance shop that had a damaged outer jacket cover and damaged inner jacket cover with exposed wiring.
