29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative: Kitchen: On, or about, February 20, 2025, the employer had not developed and implemented a lockout/tagout program that included lockout procedures and training to authorized, affected and other employees. Tasks such as, but not limited to, lubricating and cleaning/sanitizing were performed on equipment without implementing lockout procedures and providing training to employees.

29 CFR 1910.335(a)(1)(i):Employees working in areas where there were potential electrical hazards were not provided with and were not using electrical protective equipment that was appropriate for the specific parts of the body that needed to be protected and for the work being performed: Facility: On, or about, March 26, 2025, personal protective equipment such as, but not limited to, voltage rated gloves, fire resistive clothing, face shields, arc-resistant hair nets, hard hats and hearing protection was not worn by employees who performed diagnostic testing and/or who verified that electrical equipment was properly de-energized. Abatement Note: A pair of orange rubber gloves that appeared to be voltage rated were not marked to indicate that they met industry recognized standards for personal protective equipment.
