29 CFR 1926.303(b)(1): Grinding machines were not equipped with safety guards in conformance with the requirements of ANSI B7.1-1970, Safety Code for the Use, Care, and Protection of abrasive wheels, and paragraph (d) of this section. Constructing the faletali malo columns: The electric grinder used to cut pieces of rebar was missing a guard exposing employees to struck-by hazards.

29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems. Constructing the faletali malo columns: Employees were working at the edge, measuring 7.5 feet above ground level, without fall protection. Employees were exposed to fall hazards.
