29 CFR 1926.454(a):The employer did not have each employee who performed work while on a scaffold trained by a person qualified in the subject matter to recognize the hazards associated with the type of scaffold being used and to understand the procedures to control or minimize those hazards. a) For the construction site adjacent to 11169 Trinity Blvd., Trinity, FL, on or about 03/07/2025, the employer exposed employees to fall hazards of approximately 14 feet, in that, employees working off of a fabricated frame scaffold had not been trained to recognize fall hazards associated with scaffolding. Employees were not aware of the height at which guard rails were required for the scaffolding, and multiple guardrails were missing at the working level.
